If you prefer to fill out forms in the office, please arrive 20 minutes before your appointment. You can easily complete forms on our office iPad and enjoy a cup of coffee.
*If you have had recent x-rays taken (within the past 12 months), you can request a copy of your records and bring them along with you to your appointment. If they are digital x-rays, please send them to us prior to your first visit [email protected] (individual JPEG images are best).
