If you would like to fill out forms in the office, please arrive 30 minutes before your appointment.  You can easily complete forms on our office iPad and enjoy a cup of coffee.  

*If you have had recent x-rays taken (within the past 12 months), per your requestion, your previous office can forward digital x-rays to us prior to your initial visit.   Please have them emailed to:   [email protected] (individual JPEG images are best for image quality).
